Problem Statement
Using the Sensely Conversational platform clinical teams are able to gather daily medical data from patients in their homes.  How do we present the data to help clinical staff to evaluate the patient's needs and progress?  How data-dense should we present the medical records?  What kinds of interventions and actions should we enable for clinicians for each medical alert?
Users
All members of the Clinical Care team - Doctors, Nurses, Hospital Administrators.  Demographics: Primary User - 25-60 year old post secondary educated professionals.

Conversations and Design Reviews with Medical Doctors, Nurses and Subject Matter Experts on the Sensely Clinical Staff.

Designs
Clinicians review a list of their assigned patient within a cohort. 
Organizing their daily workflow, clinicians can sort the patient views based upon the alert levels or types.
Default Sort - Red Alerts, patients with alerts that are medical desire will appear at the top of the view.

Patients have the option to request a call back from the medical care team. These alerts are raised to the team by the patient.

Clinicians are often assigned more the 1 cohort of patients or wish view a subset of the patients prescribed unique checkins.
Hovering over a patient row provides details of about each raised alert.  This allows the clinician to evaluate alert details without drilling into the record immediately.
Patient Medical Record View.  With vitals, care plans, team messages, and alerts configurations.  In this view the clinical team can filter/sort the patient list, drill into a patient record and take action upon any alert raised by the system.
Once a medically significant alert is raised for the patient. Clinical team members can intervene and update the outcomes onto the medical record maintained within EHR system of record.  The following interactions show examples how alerts are addressed by the clinicians.
Additional views and configurations.  During feedback sessions with Doctors, they asked for additional views and fine grain details for each of the patients vitals.  Every doctor has different areas of specialization want to only see the specific data for their speciality.  The following views offer different models laying out patient view.
